Sara has 25 years experience in Social Work, specializing in helping adults with complex needs. Sara is skilled in navigating the legal system, obtaining insurance benefits, ethically making complex medical decisions, and managing care transitions. Consulting Services

Sara Vazquez has 25 years experience as a Social Worker, working within complex State agencies, the courts, hospitals, rehab facilities, nursing homes, and providing Hospice care.

Sara, offers consulting services to assist aging adults and their families with navigating the care continuum. Services include an initial assessment of need, to determine how best to support the aging adult and their families. Next, an individualized care plan will be created, focusing on clear goals and assisting to coordinate needed services or placements. The goals of service are to keep aging adults safe, preserve independence and dignity, and to promote self-determination.

Sara is a certified mediator and uses these skills to assist families with understanding how best to care for their loved ones and to find common ground when difficult decisions need to be made.

Sara also offers consulting services to Attorneys who may have clients or cases with complex social services needs.
